Mommy makeover with BBL: can you combine them, why many surgeons stage them, fat embolism and clot risks, 360 options, cost and recovery.
A mommy makeover can be combined with a Brazilian butt lift (BBL), but many plastic surgeons advise caution or a staged plan. The BBL uses fat from the liposuction part of the mommy makeover to add volume to the buttocks, which shapes a narrower waist — yet the two procedures need opposite recovery positions, and together they can stretch surgery to 6–10 hours, raising the risk of blood clots and fat embolism.

Mommy makeover with BBL — the short answer

Yes, a mommy makeover can be combined with a BBL in one operation, but many plastic surgeons advise against it or approach it with great caution. Combining them means one anaesthetic and one recovery, and the liposuction already planned for the waist provides the fat for the buttocks. The cost is a much longer operation and two recovery rules that pull in opposite directions.
No — a standard mommy makeover does not include a BBL. Its core is a tummy tuck, breast surgery and liposuction; the American Society of Plastic Surgeons lists buttock augmentation only as one of the procedures a mommy makeover may include. A BBL is an add-on that has to be planned, priced and risk-assessed separately. For the core procedures, see what a mommy makeover is.

The two procedures carry different serious risks. A 2021 review of the literature concluded:
"Abdominoplasty and BBL appear to have similar safety based on mortality; however, the nature of their mortalities is different." — Del Vecchio DA et al., Plast Reconstr Surg 2021;148(6):1270–1277
Most tummy tuck deaths are caused by deep vein thrombosis and pulmonary embolism, while BBL deaths are linked to fat entering blood vessels and travelling to the lungs. The review reports a BBL mortality of about 1 in 15,000 in recent surveys, down from an earlier figure of 1 in 3,000. Plastic surgery societies have issued an urgent warning about BBL risks, and the ASPS lists deep vein thrombosis, cardiac and pulmonary complications and fat necrosis among them. Adding procedures also raises complications in general: in 25,478 tummy tuck patients, combining it with liposuction and other body contouring brought major complications to 10.4% (Winocour et al., 2015). More on overall risk: mommy makeover death rate.
| Mommy makeover with BBL plan | Combined (one operation) | Staged (two operations) |
|---|---|---|
| Anaesthetics | One | Two |
| Operating time | Often 6–10 hours | Shorter sessions |
| Recovery positions | Conflicting (no face-down, no sitting on buttocks) | One set of rules at a time |
| Overall downtime | One recovery period | Two recovery periods |
| Typical approach | Selected healthy patients | Preferred by many surgeons; or a small fat transfer to the hips only |

A mommy makeover with a BBL typically costs $20,000 to $40,000 or more in the US, with comprehensive plans averaging around $30,000 to $35,000. In Turkey, combined packages are all-inclusive and lower.

A 360 mommy makeover treats the whole midsection circumferentially — abdomen, flanks and back — together with the breasts in one session. It pairs a tummy tuck with 360° liposuction, and sometimes a fat transfer. Livist Medical's 360° tummy tuck + 360° liposuction + breast lift package is €7,290 (≈ $8,183). Liposuction alone is covered in lipo 360 cost.
If loose skin and separated muscles are the main problem, liposuction or a BBL alone will not fix them.
Recovery from a mommy makeover with BBL is more demanding than either procedure alone. You will rest and sleep as your surgeon instructs, usually on your side with hips slightly bent and pillows supporting you, avoiding direct pressure on the buttocks, wear compression, walk briefly from the first day and follow your surgeon's sitting rules for several weeks. Over time, about 60–70% of the transferred fat typically establishes a blood supply and stays, behaving like normal body fat if your weight changes. The general timeline is in mommy makeover recovery time.
Whether a BBL belongs in the same operation as your mommy makeover is a safety decision first and a design decision second. Livist Medical arranges BBL and combined mommy makeover packages at contracted healthcare institutions holding a Health Tourism Authorization Certificate.
